Note: We will process your request for the Transcript of Records copy for the requesting School/University to be placed in an envelope with the PUP letterhead and entrust the sealed envelope to your representative for mailing.

1. Request letter coming from the School/University that requires you to submit Transcript of Records or Diploma or the school where you are admitted;
2. Two (2) valid Identification Card with picture and signature;
3. Two (2) pieces of 2x2 inch. ID picture (on white background;recent, not more than three months) of the requesting person;
4. Duly Accomplished General Clearance Form;
5. Payment:
   • Non-engineering graduates: Three Hundred Fifty Pesos (PhP 350.00)
   • Engineering graduates: Four Hundred Fifty Pesos (PhP 450.00)
   • Non-graduates: One Hundred Pesos (PhP 100.00) per page
   • Two Hundred Pesos (PhP 200.00) per set of diploma request.
6. Payment of One Hundred Fifty Pesos (PhP 150.00) for Honorable Dismissal

1. Request letter coming from the School/University that requires you to submit Transcript of Records or Diploma or the school where you are admitted;
2. Special Power of Attorney notarized at the place of your residence abroad with one (1) piece 2x2 inch. ID picture of representative (on white background; recent, not more than three months);
3. Photocopy of your Passport with picture and signature;
4. Representative's two (2) valid Identification Card with picture and signature;
5. Two (2) pieces of 2x2 inch. ID picture (on white background; recent, not more than three months) of the requesting person;
6. Letter of the requesting person stating specific details/instructions;
7. Duly Accomplished General Clearance Form;
8. Payment:
   • Non-engineering graduates: Three Hundred Fifty Pesos (PhP 350.00)
   • Engineering graduates: Four Hundred Fifty Pesos (PhP 450.00)
   • Non-graduates: One Hundred Pesos (PhP 100.00) per page
